Understanding your insurance coverage is crucial if you are relying on it to cover/reimburse you for the cost of our services. Here’s what you need to know:
Bring Your Insurance Card
On your first visit with us, be sure to bring your current insurance card. Please notify us of any changes in your coverage or your contact address. Covered benefits and deductibles frequently change, so it is important that you confirm these particulars with your health plan/insurance company. Contact your insurance carrier directly using the number on your insurance card to confirm this.
Our Commitment To You
We will electronically transmit accurate claims to your insurance company soon after your visit in order to encourage their fastest payment/reimbursement response. We will back this up with proper medical documentation when requested. There are no guarantees when it comes to payments from insurance companies. Promises made by some of them are not always promises that they keep. This is why we must ask, as a general policy, for your payment of our fees at the time they are provided.

Managed Care Contracts
We are not contracted with any managed care/HMO plans, including Kaiser. We are not currently contracted with Medicare.
Important Note On Referrals
If your plan requires a referral from a Primary Care Physician, please be sure to present it on your first visit.
