A Conservative-First Approach to Painful Shoulder Conditions
Most painful shoulder conditions do not require surgery as a first step. When the injury is atraumatic, degenerative, partial-thickness, chronic, or driven by overload rather than acute structural failure, the evidence supports a conservative-first plan: accurate diagnosis, progressive exercise and loading, mobility work, and scapular and thoracic retraining, with manual therapy and chiropractic-style techniques used as adjuncts.
What drives shoulder pain is rarely “damage” alone. It is usually tendon overload, capsular stiffness, poor scapular control, thoracic stiffness, sensitized pain pathways, muscle guarding, sleep disruption, and deconditioning. Each of these is modifiable — and most respond to the right protocol without an operating room.

Shoulder Conditions We Treat

Rotator Cuff Tendinopathy & Tears
Partial-thickness, small full-thickness, and many degenerative tears respond to progressive loading and manual therapy. Imaging that shows a tear does not automatically mean surgery.
Frozen Shoulder (Adhesive Capsulitis)
Most cases resolve with stage-matched mobility work and graded loading. Stretching intensity must match irritability — too aggressive in the freezing stage flares pain and sleep.
Subacromial Impingement
Pain is rarely about “bone rubbing on tendon.” It is usually a modifiable load and movement disorder of the cuff, scapula, and thoracic spine.
Biceps Tendinitis
The long head of the biceps is sensitive to scapular position, cuff weakness, and overhead loading. Conservative care reduces provocative load and restores anterior shoulder mechanics.
Atraumatic Instability & Subluxation
Driven by poor neuromuscular control, laxity, and proprioceptive deficits — rehabilitation is first-line, not surgery.
Shoulder Osteoarthritis
Pain and function often improve with mobility work, strengthening, sleep positioning, and load management — even when the joint shows degeneration on imaging.

1Sports Chiropractic & Manual Therapy
Glenohumeral mobilization, thoracic spine assessment, soft-tissue work, and scapulothoracic mobilization. Used as an adjunct to active rehabilitation — not a stand-alone cure. The literature supports manual therapy combined with exercise for cuff pain and adhesive capsulitis.
2Supervised Strength Rehabilitation
Progressive loading of the rotator cuff, scapular stabilizers, posterior shoulder, thoracic extensors, and kinetic chain. This is the most heavily evidenced conservative intervention and is delivered in a facility built for it — not in a hallway between adjustments.
3Postural & Scapular Retraining
The shoulder is not just the ball and socket. The glenoid sits on the scapula, the scapula sits on the rib cage. We restore variable, efficient positions — thoracic extension, scapular control, rib-cage mobility — so the cuff does not have to overwork to center the humeral head.
4Assisted Stretching & Mobility
Targeted at posterior capsule, pectoralis minor, latissimus, and thoracic stiffness. In frozen shoulder, dosage is matched to stage: gentler pain-limited mobility early, more assertive stretching as irritability decreases.
5Nutritional & Recovery Guidance
Tendons, cartilage, and bone respond poorly to systemic inflammation, low protein intake, poor glycemic control, vitamin D deficiency, and inadequate recovery. Adhesive capsulitis in particular is closely linked to diabetes. Framed as supportive care — not a cure for structural injury.
Why Small Rotator Cuff Tears Sometimes Hurt More Than Large Ones
This surprises most patients, but it is well-known in shoulder medicine: pain intensity does not correlate cleanly with tear size on imaging. A small or partial tear can be sharply painful while a large chronic tear is less symptomatic. Several mechanisms explain it:
- Small tears retain tension. Remaining fibers are still loaded, especially when the arm is compressed or internally rotated during sleep.
- Partial-thickness and bursal-sided tears are chemically irritable. Pain comes from inflammatory mediators, bursitis, and synovitis — not just the tear itself.
- Large chronic tears can decompress. A retracted tendon may produce weakness without focal tension pain.
- Night pain is position-sensitive. A small tear in a painful arc can flare badly with side-lying, regardless of structural size.
- Pain is neurophysiological, not purely anatomical. Sleep loss, anxiety, central sensitization, and guarding amplify pain independent of MRI severity.
This is why the clinical question is never just “How big is the tear?” — it is: Is it traumatic or degenerative? Is strength preserved? Is function declining? Is pain modifiable? Is the patient improving with structured care? Those are the questions that determine whether surgery is appropriate.
Why Shoulder Pain Wrecks Sleep — and How We Help
Side-lying compresses the painful shoulder, narrows the available joint space, tensions the capsule, and loads the cuff and biceps tendon. Frozen shoulder is especially notorious for night pain. Rotator cuff disease consistently disrupts sleep, particularly when lying on the affected side or when the arm falls into extension, adduction, or internal rotation.
Poor sleep then makes pain worse through central sensitization, reduced pain inhibition, fatigue, mood changes, and impaired tissue recovery — creating the cycle most shoulder patients describe:
Shoulder pain → poor sleep → increased pain sensitivity → more guarding → worse movement → more pain.
Practical sleep modifications are a routine part of our care plans: sleeping on the opposite side with a pillow supporting the painful arm, avoiding the arm hanging across the body, using a reclined position during severe night pain, and supporting the elbow and forearm to reduce traction on the cuff and biceps. Breaking the cycle is often the single fastest way to reduce daytime pain.
When Conservative Care Is the Right Call — and When It Isn’t
Surgery has a real role. We are not anti-surgical; we are evidence-aligned. We refer when the clinical picture warrants it.
Conservative care is usually appropriate when:
- The tear is atraumatic or degenerative, especially in older adults
- The tear is partial-thickness, or a small full-thickness tear without major weakness
- Active elevation and function are preserved
- Symptoms are improving with structured rehabilitation
- Surgical risk is high or functional demand is modest
- Frozen shoulder co-exists with cuff pathology (operating into a stiff, irritable shoulder complicates recovery)
Surgical consultation is appropriate when:
- There is an acute traumatic full-thickness tear with sudden weakness
- Pseudoparalysis or major functional collapse is present
- Progressive neurological signs, fracture, infection, or tumor are suspected
- Recurrent traumatic instability has failed structured rehabilitation
- A well-performed conservative program has failed to produce meaningful gain
For many degenerative rotator cuff tears, systematic reviews and meta-analyses have found that surgery is not clearly superior to active physiotherapy for pain and function. That does not mean surgery is never right — it means imaging showing a tear is not, by itself, a reason to operate.

Often, no. For atraumatic and degenerative rotator cuff tears, multiple systematic reviews show conservative
care produces outcomes comparable to surgery for pain and function. Partial-thickness tears and small
full-thickness tears are usually managed conservatively first. Surgery becomes the right call for acute traumatic
tears with sudden weakness, pseudoparalysis, progressive functional loss, or after a well-run conservative
program has failed.
Can frozen shoulder be treated without surgery?
Yes — and most cases should be. Adhesive capsulitis responds well to stage-matched manual therapy,
graded mobility, and progressive strengthening. The key is calibrating intensity to irritability: aggressive
stretching during the painful freezing stage flares symptoms and wrecks sleep. Dr. Cancel has extensive

Why do small rotator cuff tears sometimes hurt more than large ones?
Pain does not correlate cleanly with tear size. Small or partial tears retain tendon tension and are often

dictates the pace. Chronic cases, post-surgical cases, and cases complicated by sleep disruption or central

Do you treat shoulder injuries after other care has failed?
Yes — this is a large share of our caseload. Patients commonly arrive after adjustment-only chiropractic,
generic physical therapy, or imaging-driven surgical consultations did not produce results. The most common
reason for failure is that the actual driver of pain — scapular dyskinesis, thoracic stiffness, load intolerance, or
