Frequently Asked Questions
Sports chiropractic care at Performance Sport Care in Walnut Creek — answered by Dr. Douglas F. Cancel, D.C., a 30+ year sports chiropractor and former National-level competitive bodybuilder. Below, the questions patients most often ask before, during, and after treatment — including those from athletes, lifters, and patients who've tried other care without lasting relief.
About Our Practice
What is Performance Sport Care, and how does it differ from many common chiropractic clinics?
Performance Sport Care is a sports chiropractic and rehabilitation clinic — not a general adjustment-only or subluxation-based practice. Dr. Douglas F. Cancel, D.C., a former National-level competitive bodybuilder and lifelong strength-training athlete with more than 30 years of clinical experience, begins every case with a careful, accurate diagnosis before any treatment plan is built. From there, he applies a full spectrum of professional sports medicine modalities — manual therapy, soft tissue work, joint mobilization, and supervised progressive strength training — making elite-level care available not just to athletes, but to anyone who wants the fastest and most durable improvement in the shortest possible time. Our facility includes a rehabilitative strength training center that is second to none in the Walnut Creek community, equipped with some of the same equipment used by NFL teams and adapted for patients of nearly every experience and ability level.
Who does Dr. Cancel treat?
Dr. Cancel treats the full activity spectrum — competitive bodybuilders, powerlifters, weightlifters, CrossFit athletes, runners, recreational lifters, weekend athletes, desk workers with chronic neck or back pain, post-surgical recovery patients, and older adults working to preserve strength and mobility. The same principles that keep professional and competitive athletes performing — accurate diagnosis, targeted manual therapy, and progressive strength-based rehabilitation — apply to anyone who wants to move better, hurt less, and stay active for the long term. Our rehabilitation facility makes it possible to bridge the gap between pain relief and full functional recovery for virtually any patient.
Does Dr. Cancel treat bodybuilders, powerlifters, and other strength athletes?
Yes — and this is one of the clinic’s specialty areas. Dr. Cancel is a former National-level competitive bodybuilder and a lifelong strength-training athlete, which means he understands strength-sport injuries from the inside — the volume, the fatigue, and the way heavy squats, presses, and pulls load the spine, shoulders, hips, and knees. Care for strength athletes includes assessment based on how you actually train, manual therapy targeted to the tissues under the highest load in your sport, rehabilitation designed to keep you in the gym whenever possible, and programming guidance on load management during recovery. The clinic serves bodybuilders, powerlifters, weightlifters, and CrossFit athletes throughout Walnut Creek, Lafayette, Danville, and Concord.
I have already tried chiropractic care, physical therapy, or injections without lasting relief. Can Dr. Cancel still help?
Yes — in fact, this describes a large portion of our patients. When generic chiropractic adjustments, standard physical therapy, or cortisone injections fail to produce a lasting result, the most common reason is that the original diagnosis was incomplete or incorrect. Dr. Cancel’s first step with a new patient is always a careful, accurate diagnosis: identifying the actual pain generator, the movement dysfunction that caused it, and the tissue that needs to be rebuilt. Once the diagnosis is right, the treatment plan becomes targeted rather than generic, and recovery typically follows. Many patients arrive frustrated after months or years of partial results elsewhere and experience meaningful relief within the first few visits.
What does "subluxation-based" chiropractic mean, and how is your approach different?
Subluxation-based chiropractic is a model in which spinal adjustments are the primary, often the only, intervention — and adjustments are repeated visit after visit without a comprehensive diagnostic workup or active rehabilitation. Many patients improve briefly and then plateau because the underlying movement dysfunction and tissue issues that caused the problem are never addressed. Performance Sport Care is fundamentally different: a thorough diagnostic evaluation comes first, manual therapy is targeted rather than generalized, and supervised progressive rehabilitation is built into the plan from day one. Adjustments are one tool of many, used when they are the right tool — not as a default response to every visit.
Where is Performance Sport Care located?

Services & Treatment
What conditions does Performance Sport Care treat?
We treat a wide range of musculoskeletal and pain conditions, including:
- Muscle strains and muscle spasms
- Back and neck pain
- Shoulder, knee, hip, and foot/ankle injuries
- Tendinitis and elbow pain, and overuse injuries
- Sciatica and disc-related pain
- Tension headaches and cervicogenic pain
- Post-surgical rehabilitation support
- Poor posture and movement dysfunction
- Chronic pain of musculoskeletal origin
What treatment methods does Dr. Cancel use?
Dr. Cancel applies a comprehensive spectrum of modalities drawn from professional sports medicine — chiropractic adjustments, myofascial release, instrument-assisted soft tissue mobilization, deep tissue and trigger point therapy, joint mobilization, assisted stretching, corrective exercise, movement coaching, and supervised progressive rehabilitative strength training in our on-site facility. Because every treatment plan begins with an accurate diagnosis rather than a generic protocol, the modalities used in your specific case are precisely targeted to the pain generator and movement dysfunction we identify — so you experience meaningful results in as few visits as possible.
Do you treat both acute (sudden) injuries and chronic (long-term) conditions?
Yes. We treat both acute injuries — such as sprains, strains, and sudden-onset pain — and chronic conditions caused by repetitive stress, postural imbalances, or long-standing dysfunction. Many painful conditions, when caught early and treated correctly, can resolve in a matter of days. More complex or chronic cases may require a longer course of care along with active patient participation in recommended nutrition, exercise, lifestyle, and activity protocols.
My MRI says my back, neck, or joint looks normal but I am still in pain. What is going on?
A “normal” MRI rules out major structural damage — but it cannot evaluate movement quality, joint mechanics, motor control, or functional muscle performance. Many chronic pain drivers show up nowhere on an MRI report, including multifidus dysfunction (the small, deep stabilizer muscles of the spine), faulty movement patterns, segmental instability, and soft-tissue restrictions. Dr. Cancel’s diagnostic process is built specifically to find what imaging misses: a detailed history, neurological screening, orthopedic testing, palpation, and a movement-based functional assessment, with imaging findings integrated rather than relied on in isolation. For more on this, see our blog post: When Your MRI Looks ‘Normal’ But You Still Hurt.
Tell me about your rehabilitation and strength training facility.
Our rehabilitative strength training facility is small but is likely the most advanced of any chiropractic office in the Walnut Creek community. We use some of the same equipment found in NFL training facilities, and we are skilled at adapting these tools across virtually every level of fitness, ability, and experience — from competitive bodybuilders, powerlifters, and CrossFit athletes through to patients returning to movement after years of inactivity or injury. The facility allows us to take recovery beyond passive pain relief and into supervised progressive loading, multifidus and stabilizer activation, and sport- or work-specific functional restoration in the same visit. This is uncommon in East Bay chiropractic offices and is one of the primary reasons patients achieve outcomes here that they could not achieve elsewhere.
Can chiropractic care improve athletic performance, not just treat injuries?
Absolutely. Many athletes use chiropractic care proactively to optimize joint function, improve mobility, reduce muscle tension, and enhance neuromuscular coordination. Regular maintenance care may help you perform at a higher level while reducing your risk of future injury. But you don’t need to be an athlete to benefit — these same performance principles apply to anyone who wants to move and feel better in daily life.
Your First Visit
What should I expect at my first appointment with Dr. Cancel?
Your first visit begins with a thorough intake and health history review, followed by a detailed postural, neurological, orthopedic, and movement-based functional assessment. Dr. Cancel’s diagnostic approach goes well beyond surface-level symptom management — the goal is to identify what’s actually driving your condition so that treatment is precise and durable from the first visit forward. You will leave your first appointment with a clear explanation of what is actually wrong, an accurate diagnosis, and a personalized treatment plan — and in most cases, treatment will begin during that same visit.
How long does a first appointment take?
Initial appointments typically run 45–60 minutes to allow adequate time for comprehensive evaluation and to begin your first treatment. Follow-up appointments are a minimum of 30 minutes — longer than the industry standard — because Dr. Cancel believes that meaningful, lasting care cannot be rushed.
Do I need a referral to see Dr. Cancel?
A: No referral is necessary. You can contact Performance Sport Care directly to schedule your appointment. We also welcome referrals from primary care physicians, orthopedic surgeons, physical therapists, and coaches.
What should I bring to my first visit?
Please bring a valid photo ID, your insurance card (if applicable), any relevant imaging such as X-rays or MRI reports, and a list of current medications or supplements. Wearing comfortable, athletic clothing that allows easy access to the affected area is also recommended.
Treatment Plans & Recovery
How many visits will I need?
The number of visits depends on the nature, severity, and duration of your condition, as well as your individual response to treatment and your personal health goals. Dr. Cancel will outline a recommended care plan at your first visit and reassess regularly to track progress and adjust as needed.
How long will it take to recover?
Recovery timelines vary widely. Some painful conditions resolve in a matter of days with proper diagnosis and targeted treatment. Chronic or complex conditions may require a longer course of care. In those cases, patient participation plays a significant role — closely following Dr. Cancel’s recommended protocols for nutrition, exercise, activity modification, and lifestyle factors can meaningfully accelerate your results and the durability of your recovery.
Will I need to do anything at home between visits?
Yes — and this is important. Active patient participation is one of the most significant factors in achieving fast, lasting results. Dr. Cancel may prescribe corrective exercises, stretching routines, nutritional guidance, and lifestyle modifications to reinforce in-office treatment. Patients who follow their prescribed protocols consistently tend to recover faster, stay better longer, and require fewer total visits.
What to Expect: Honest Answers About Treatment
Will my treatments be painful?
We believe in being completely honest about this: judiciously selected and properly administered chiropractic adjustments are typically not painful and are often immediately relieving — but the soft-tissue work that follows can be initially uncomfortable in patients whose muscles have been stiff, contracted, and tender for months or years.
Many patients come to us with stiff, inflexible, contracted, and tender muscles that have been in that state for a considerable length of time. Manual therapies, deep tissue massage, stretching, and corrective exercise strategies directed at these tissues can, indeed, be initially uncomfortable or painful.
Our objective is to move patients through these initial stages as quickly and effectively as possible — so that durable relief, restored function, and lasting improvement can follow. We will always communicate what to expect, and we will never push through discomfort without clinical reason.
What does "working through the initial stages" actually mean?
It means that some of the most effective treatments — particularly those targeting chronically tight, fibrotic, or poorly conditioned soft tissues — may cause temporary soreness or discomfort before they produce relief. Think of it similarly to the way targeted physical therapy can be demanding before it becomes liberating. Dr. Cancel and our team will monitor your response closely, adjust approaches as needed, and ensure that every step of your care is purposeful and moving you forward.
General Chiropractic Questions
Is chiropractic care safe?
Yes. Chiropractic care is widely recognized as one of the safest, most effective non-invasive and drug-free approaches to musculoskeletal health. Dr. Cancel always conducts a thorough diagnostic evaluation before beginning treatment to ensure the chosen approach is appropriate and safe for your specific condition and history.
Will my adjustment hurt?
Most patients find chiropractic adjustments comfortable and often immediately relieving. Some patients experience mild soreness for 24–48 hours following their first few visits, particularly if surrounding soft tissues are tender or inflamed. For patients presenting with long-standing muscle tightness or tissue restrictions, some degree of initial discomfort during soft tissue work is possible and normal. Dr. Cancel uses a wide variety of techniques and will always tailor the approach to your comfort, tolerance, and clinical needs — and will explain what you are likely to feel before beginning any procedure.
Does every chiropractic adjustment make a "popping" sound?
No — and this is an important point. Not all chiropractic adjustments produce a popping or cracking sound, and the presence or absence of that sound is not an indicator of whether the adjustment was effective or successful. The sound, when it does occur, is called a cavitation — the release of gas bubbles from within a joint capsule, similar to cracking your knuckles. Some techniques and some joints are simply less likely to produce that sound, and treatment can be equally effective either way. Dr. Cancel will never repeat an adjustment simply to produce a sound.
Insurance & Billing
Does Performance Sport Care accept insurance?
Yes. You pay at the time of service, and we electronically submit a claim to your plan the same day so your carrier reimburses you directly. Because an accurate diagnosis typically shortens the overall course of care, most patients find their total out-of-pocket cost is lower here than with providers who rely on extended generic treatment plans. See our payment policy for full details.
What should I know about insurance coverage for Chiropractic care in California?
In California, ever-increasing deductibles, copayments, and insurance plan restrictions increasingly place the primary financial burden on patients — regardless of the provider or quality of care. We understand this is a real concern and we are fully transparent about costs. Our goal is to ensure you are never surprised by a bill and that you always understand what to expect financially before care begins.
Why is Performance Sport Care often more cost-effective compared to other options, regardless of insurance coverage?
An accurate diagnosis at the start improves the likelihood of fewer visits and a lasting result. Generic plans—chiropractic adjustments, standard physical therapy, or non-covered regenerative injections such as stem cell or PRP—often cost patients time, copays, and out-of-pocket money, particularly if the treatment turns out to be off-target. When the diagnosis is correct from the start, treatment is targeted, progress is faster, and total costs are typically lower over the full course of care, whether paid through insurance or out of pocket. That is the economic case for Performance Sport Care: better outcomes, and better outcomes per dollar spent.
Do you offer options for patients with limited insurance benefits or no insurance?
Yes. We offer transparent self-pay rates and are happy to discuss options that make care accessible. Please contact our office to learn more about current rates and what to expect financially before your first visit.
